Many companies offer matching gift programs to encourage employees to contribute to charitable organizations. Some even provide matching funds for employee volunteer hours. The impact of your gift to Pacific Primary may be doubled or possibly tripled!

Please check with your HR Department to see if they participate in matching gift programs. If they do:

1. Inform them that you wish to donate a matching gift to Pacific Primary.

2. Complete their required forms. Fill out any forms or paperwork your HR department provides. Be sure to specify Pacific Primary as the beneficiary. You will likely need to provide the following information:

• The organization’s legal name: Pacific Primary
• Tax Identification Number: 23-7373767
• Mailing Address: 1500 Grove Street San Francisco, CA 94117

3. Ask for a confirmation of your matching gift once submitted. 

4. Notify [email protected] that you have initiated a matching gift, the amount and confirmation from your employer.

2. Complete their required forms. Fill out any forms or paperwork your HR or Payroll department provides. Be sure to specify Pacific Primary as the beneficiary. You will likely need to provide the following information:

• Mailing Address: 1500 Grove Street San Francisco, CA 94117

3. Ask for a confirmation of your payroll deduction to ensure everything is set up correctly.

4. Notify [email protected] that you’ve set up a payroll deduction, the amount and confirmation from your employer.
